Part 1: Administrative Materials

£        CSP Application Six Part Folder Content Checklist

£        Attach “current” Section maps with Farm and Tract numbers from FSA showing your entire operation (outline in dark marker) 

Part 2: Self Assessment Workbook including Soil & Water Eligibility Tool (SWET) and Grazinglands Worksheets 

£     Attach completed Self Assessment Workbook

£     The following forms in the Self Assessment Workbook must be completed:

o        CCC-1200 Application form (applicant must sign front page and the last page of the appendix)

o        Landowner Consent Form (each landowner must sign a form)

£        SF 1199A Direct Deposit form (must be completed by each person who will receive payment shares.)

£        Attach additional SWET Worksheets for each rotation or permanent hayland.

Part 3: Benchmark documentation part I

£    CSP Part 3 Worksheet 1 – Fertilizer, Manure, Pesticides and Irrigation Water Records

• Attach 2007 FSA certification maps of the applicable fields

£    Example of Nutrient, Manure, etc. worksheet

£    CSP Part 3 Worksheet 2 – Grazing Lands Inventory Records

• Attach current maps of the applicable fields
